Community Integrated Services LLC (King)

A resident with mental health diagnoses exhibited escalating dangerous behaviors including suicide threats, self-harm with cuts, possession of makeshift weapons and potential explosive materials, and threatening staff and law enforcement with a shovel while other vulnerable adults were present in the home. The facility failed to report three of four serious incidents to the department as required by mandatory reporting laws, despite their own policy requiring such reports for acts causing fear of imminent harm and self-neglect. While the facility did involve law enforcement and mental health services during crises and eventually removed the resident for safety, their failure to properly notify the regulatory department represented a significant breakdown in compliance that placed all residents at ongoing risk.
