Concerning Staff and Maintenance Issues
This facility shows concerning red flags from limited available reviews. Two negative reviews highlight poor staff attitude during admissions and dirty, dingy patient rooms that need refurbishment, while one positive note simply says a visitor liked it before health improved. The small dataset and dated reviews (2012-2014, with one 2024) make it difficult to assess current conditions reliably.
Bad vibes! I went in to talk about admissions and met with a lackluster and disinterested older lady. I can guarantee that she doesn't want anyone coming to this place. Avoid this
Patient rooms small, dingy, dirty. First floor public areas much nicer than expected, was told they were recently refurbished and that the patients floors would be refurbished 'hop
Really liked it but health has since improved...
Inspections(5)
This was a routine fire safety inspection by the Washington State Fire Marshal with no violations found. The report explicitly states that all violations from previous inspections have been corrected, indicating the facility successfully addressed any prior deficiencies. The facility demonstrated compliance with fire protection requirements and maintained approval status, reflecting adequate ongoing operations and responsive corrective action from previous findings.

A valid, readable inspection report is required to perform scoring and analysis.
View original report →This investigation examined multiple allegations from a single resident regarding mistreatment, isolation, inadequate food service, and denial of medical care. The comprehensive investigation found no substantiated violations across all four complaint intakes. The facility demonstrated appropriate care practices, with documentation showing the resident had declined recommended medical treatments and frequently left the facility independently. The facility maintained proper records, posted meal service times, provided rights notifications, and offered equal services to all residents.
View original report →The May 2023 inspection identified 13 serious fire safety violations at this residential care facility, including non-functional emergency lighting in multiple locations, unsealed fire-rated penetrations, missing documentation for required fire safety systems testing (sprinklers, smoke detectors, fire doors, dampers), an overdue fire suppression system above the kitchen fryer, and failure to conduct mandated quarterly fire drills. The facility responded appropriately by correcting all violations within approximately 5 weeks, as confirmed by the July 2023 follow-up inspection showing full compliance. While the violations represented severe life-safety deficiencies that could impede evacuation and fire suppression in an emergency, the facility's timely and complete remediation demonstrates good responsiveness to regulatory requirements.
View original report →The inspection identified four compliance violations: inadequate hot water temperatures in common restrooms (59-60°F instead of required 105-120°F), missing background check documentation for two staff members in on-site files due to corporate policy change, failure to use proper DSHS-approved background check forms, and expired CPR/First Aid certifications for two resident aides. The facility responded appropriately by acknowledging all deficiencies, submitting correction plans within two weeks, and achieving full compliance verified by follow-up inspection on 05/10/2023, demonstrating effective corrective action across infection control, personnel screening, and emergency preparedness requirements.
