Beautiful Facility, Caring Staff
Merrill Gardens Auburn consistently receives praise for its beautiful, hotel-like facility with bright common areas and tasteful decor, plus exceptionally friendly and caring staff who communicate well with families. Most residents enjoy the food quality and variety of activities, though a minority cite concerns about meal service timing, blandness, and limited physical activities. One serious negative review details a failure to notify family during a medical emergency, and cost is mentioned as a barrier for some families.

Inspections(7)
Initial inspection on 4/9/2025 identified 14 fire and life safety violations including non-functioning fire doors, blocked egress paths, undercharged fire extinguishers, inoperative emergency lighting, unsecured oxygen cylinders, and missing fire drill documentation. These systemic failures in multiple fire protection systems posed serious risk to resident safety in this residential care facility. By the 8/12/2025 re-inspection, the facility corrected 12 of 14 violations, with only two fire door latching issues and one fire alarm breaker labeling deficiency remaining, demonstrating good follow-through. Final inspection on 10/20/2025 confirmed all violations corrected and facility approved.
View original report →On 10/3/2025, a stove burner was left on in a resident's apartment, causing a case of zipfizz to smolder and fill the room with smoke. The facility's smoke detection system alerted staff immediately, caregivers responded promptly to remove the hazard and ventilate the room, and the resident was safely relocated. The facility demonstrated good response protocols with immediate staff response and proper notification systems, though the incident reveals moderate concerns about resident supervision and stove safety given the resident had no recollection of turning on the burner. No International Fire Code violations were found during the follow-up inspection on 10/21/2025.
View original report →Initial inspection on 04/09/2025 revealed severe and systemic fire safety violations at this residential care facility, including multiple fire doors that failed to latch, non-functional emergency lighting in stairwells, improper oxygen cylinder storage, blocked egress paths, fire-rated penetrations, and inadequate fire alarm circuit labeling—all creating substantial life-safety risks for vulnerable residents. The facility's response was moderately adequate: most violations (10 of 14) were corrected by the 08/12/2025 re-inspection, but critical issues remained unresolved including fire doors still failing to latch in room 320 and corridor areas, and missing fire alarm circuit labels, resulting in disapproval status. While the facility demonstrated effort to address most deficiencies within the reinspection timeframe, the persistence of life-safety violations involving fire door integrity indicates incomplete corrective action and inadequate quality control in a high-risk residential environment serving elderly residents.
View original report →This facility had severe repeat violations including failure to follow food sanitation protocols putting 47 residents at risk of foodborne illness, and failure to ensure proper nurse delegation services for two residents risking improper medication administration. Both deficiencies were previously cited in November 2024 and remained uncorrected at the January 2025 follow-up inspection, demonstrating a pattern of non-compliance. The facility's response was inadequate, as evidenced by the repeat citations and imposition of $900 in civil fines for failing to implement effective corrective actions after the initial violations.
View original report →The facility had multiple recurring violations across food safety (improper handwashing, temperature control failures), medication management (undelegated staff administering medications, expired medications), and safety issues (unsecured medication rooms, inadequate ventilation, hazardous equipment access). While initial violations in October-November 2024 were moderate in severity, some deficiencies recurred in December 2024 despite corrective action plans. The facility demonstrated a good response by implementing corrections that resulted in full compliance by January 28, 2025, with no deficiencies found on follow-up inspection, though the pattern of recurrence indicates systemic monitoring gaps that required regulatory intervention to fully address.
View original report →The May 2024 inspection identified 15 life-safety violations at this residential care facility, including failed fire dampers, overdue sprinkler system testing, non-compliant fire doors, breached fire walls, and blocked exits - creating serious fire safety risks for vulnerable residents. The facility responded appropriately by conducting necessary inspections, repairs, and corrective actions, as evidenced by the September 2024 follow-up inspection confirming all violations were corrected within the 4-month timeframe. While the initial violations were severe and systemic, affecting multiple critical fire protection systems, the facility demonstrated good corrective action by addressing all deficiencies and achieving full compliance.
View original report →The March 2023 inspection identified 16 fire and life safety code violations at this residential care facility, including missing documentation for required testing (fire drills, emergency lighting, CO detectors, kitchen suppression system, fire dampers), electrical safety issues (extension cords, missing outlet covers, multi-plug adapters), fire door failures, fire wall penetrations, and an unsecured oxygen tank. The facility responded appropriately by correcting all violations by the April 2023 follow-up inspection, demonstrating effective remediation of systemic documentation gaps and physical safety deficiencies. While the violations represented a pattern of non-compliance across maintenance, testing documentation, and safety systems with potential for resident harm, none resulted in actual incidents and the facility's timely comprehensive correction demonstrates adequate commitment to regulatory compliance and resident safety.
