Warm Community, Stretched Resources
Ballard Landmark receives consistently high praise for its friendly, caring staff, abundant activities, excellent location in Ballard, and bright, clean modern facilities. Residents and families appreciate the strong sense of community, wellness programs including a saltwater pool, and the quality of life improvements residents experience. However, concerns emerge around understaffing that stretches caregivers thin, inconsistent administrative follow-through on billing and communication issues, occasional housekeeping problems, and the facility being better suited for lighter-level assisted living rather than high-care needs.
The nutrition and exercise program were outstanding. The rooms were small and dull. The entry halls were quite noisy.
The residents clearly had a genuine appreciation of each other. They were happy with the activities, their apartments, the food, the caregivers, and the location. The common space

The residents are super-friendly on the whole. It is impossible to have a ride in the elevator without a welcoming greeting from someone. The facility/staff are mostly accessible a
Inspections(6)
The November 2025 inspection identified 10 fire and life safety code violations at this residential care facility, including exposed electrical wiring, improper extension cord use, missing fire system documentation, non-functional emergency lighting, fire door deficiencies, and multiple fire-rated wall penetrations. The violations primarily involved building systems maintenance and documentation gaps rather than immediate life-threatening conditions. The facility responded appropriately by addressing all violations, as confirmed by the February 2026 follow-up inspection showing full compliance and approved status within approximately three months.
View original report →The inspection identified three moderate violations: failure to assess safety risks associated with a resident's bed rail use, missing veterinary certifications for facility pets, and incomplete tuberculosis testing for a newly hired staff member. The facility acknowledged all deficiencies and provided attestation of corrective actions with a completion date of 4/17/2025. A follow-up inspection on 4/23/2025 confirmed all deficiencies were corrected and the facility met all licensing requirements, demonstrating good follow-through on compliance.
View original report →The November 2024 inspection identified 14 violations at this residential care facility, primarily involving fire safety systems maintenance (missing inspection records for sprinklers, fire dampers, emergency lighting), exit pathway obstructions (mobility scooters blocking corridors), and commercial kitchen safety issues (excessive grease accumulation, inaccessible hood ducting). The facility demonstrated a good response by correcting all violations between the November inspection and the February 2025 follow-up, achieving full compliance and approved status. While the violations represented significant fire code non-compliance affecting multiple safety systems, none posed immediate life-threatening conditions, and the facility's timely remediation of all issues demonstrates appropriate corrective action.
View original report →The October 2023 inspection identified 10 fire safety violations including blocked egress, non-functioning fire doors at multiple locations, missing documentation for required safety system inspections (hood cleaning, CO alarms, emergency lighting, fire dampers), and compromised fire-rated construction in electrical rooms. The facility responded appropriately by correcting all violations within approximately two months, as confirmed by the November 2023 follow-up inspection showing full compliance. While the violations represented a pattern of maintenance and documentation deficiencies affecting fire safety systems, none created immediate life-threatening conditions, and the facility's timely remediation demonstrated adequate commitment to regulatory compliance.
View original report →A resident was given another resident's sedating medication, resulting in a fall, hospitalization with high blood sugar, dizziness, and altered mental status. The medication error prevented an MRI due to heavy sedation. The facility was cited for failing to maintain a safe medication delivery system (WAC 388-78A-2210). The facility conducted appropriate family notification regarding the hospitalization but did not proactively disclose the medication error itself; the Director of Nursing defended current assessment practices but acknowledgment of the systemic medication safety failure was limited.
View original report →The January 2023 inspection identified 15 violations at Ballard Landmark Inn, primarily consisting of missing documentation for required maintenance and testing (fire alarm, sprinkler, kitchen suppression systems, generator logs) and lack of emergency evacuation plans. Additional issues included blocked electrical panels, missing outlet covers, damaged fire walls, and dust-covered sprinkler heads. The facility demonstrated a good response by correcting all violations within two months, as confirmed by the March 2023 follow-up inspection showing full compliance. While the violations were widespread across multiple fire safety systems, none posed immediate life-threatening conditions, and the timely remediation indicates appropriate facility responsiveness to regulatory requirements.
