Warm Staff, Maintenance Concerns

Village Green (formerly Daystar) earns consistent praise for its warm, family-like atmosphere and caring staff who know residents by name and provide dedicated support. However, serious concerns emerge around medical staffing competency, facility maintenance (older building with ant infestations reported), and inconsistent communication, with some families experiencing care mismatches and unreturned calls. The convenient West Seattle location near shopping appeals to many, though experiences vary significantly depending on care level needs.

I would recommend to others. The staff were all friendly and supportive of my dad. He was taken care of and staff made an effort to keep him engaged. Location was a big attraction.

The inspection identified multiple fire and life safety code violations including blocked fire doors, missing maintenance records for sprinkler and fire alarm systems, inadequate emergency lighting testing, and incomplete fire drill documentation. The facility demonstrated a good response by correcting the blocked door violation during the re-inspection and addressing fire drill documentation issues, though several maintenance and testing requirements remain outstanding. The violations represent moderate severity as they involve systemic gaps in safety system maintenance and documentation rather than immediate life-threatening conditions. The facility's prompt correction of immediately observable violations and cooperation with inspectors reflects adequate responsiveness to identified deficiencies.

The facility had two administrative violations: one staff member's tuberculosis skin tests were spaced 33 days apart instead of the required 7-21 days, and health certifications for 2 of 3 pets were missing. The Executive Director acknowledged the TB testing finding during the inspection, but despite being given additional time until the deadline, failed to obtain the required veterinary certifications for two pets before the compliance deadline. While these are procedural violations with no evidence of actual resident harm, the incomplete response to the pet certification requirement demonstrates gaps in follow-through on corrective actions.

The facility failed to deliver prescribed medication to a resident for four days following hospital discharge, despite the pharmacy delivering it to the facility on 10/12/2024. The resident was documented as unstable and unpredictable post-hospitalization, yet no proper assessment or discharge planning was completed, and no family medication assistance agreement was in place. The facility's response was minimal - while they eventually delivered the medication on 10/16/2024, there is no documented investigation, corrective action plan, or systemic changes to prevent similar medication delivery failures.

The inspection identified multiple fire safety and code compliance violations including improper storage blocking fire safety equipment, missing safety documentation (fire door inspections, fire/smoke damper testing, quarterly sprinkler inspections), electrical hazards (extension cords, missing junction box covers), compromised fire-rated construction, non-compliant egress gate hardware, and missing circuit breaker lock on fire alarm system. The facility responded appropriately by scheduling corrections and conducting a follow-up inspection on 7/27/2023 that confirmed all violations were corrected, demonstrating timely remediation of the identified deficiencies. While violations affected multiple critical fire safety systems, none posed immediate life-threatening danger, and the facility's prompt corrective action within approximately one month showed good compliance commitment.
