Reviews
Exceptional Staff, Outstanding Care
Merrill Gardens Ballard earns exceptional marks for its welcoming staff, immaculate facilities, and restaurant-quality food served on a flexible schedule. Families consistently praise Community Relations Director Ian Mackenzie for honest guidance and the long-tenured staff for genuine care, while residents enjoy diverse activities, spacious apartments with modern amenities including in-unit washers/dryers, and a prime walkable Ballard location. The sole concern raised by one family was inadequate nighttime staffing for memory care residents, particularly those with neurodegenerative diseases, though the facility excels for independent seniors.

View original report →The facility failed to develop adequate care plans for Resident 3's continuous glucose monitoring, with no documented roles for family members providing supplies or backup plans if supplies were unavailable. Additionally, Resident 4 was assessed as high fall risk but the report cuts off before detailing related deficiencies. The facility was given 45 days to correct deficiencies and had consultation provided on water temperature requirements. Response was moderate as the facility cooperated with the inspection and was provided time to develop corrective action plans, though no evidence of immediate corrective actions is documented in this initial deficiency report.
