The facility failed a fire marshal inspection on 7/24/2025 and re-inspection on 9/8/2025 due to missing documentation for forward flow testing, fire alarm system maintenance, an unlocked circuit breaker, and emergency generator testing records. These were routine fire safety documentation and maintenance issues, not active system failures posing immediate danger. The facility responded promptly by correcting several items within days (circuit breaker locked 9/11, generator testing 9/12, forward flow completed 8/5) and scheduling the annual fire alarm inspection by 10/1/2025, with all corrections verified as complete by the 11/10/2025 follow-up inspection.

The vast majority of violations were corrected on-site during the inspection, demonstrating immediate responsiveness. The few remaining uncorrected items at the follow-up inspection (fire alarm documentation, generator testing records) were subsequently resolved, reflecting strong facility commitment to compliance and resident safety.
